Marine report: Pilot errors, lack of training and flight time caused crash

Released to The San Diego Union-Tribune this past week following a Freedom of Information Act request, the thick investigative report appears to have been finished on Jan. 17.

In his letter endorsing the bulk of the findings in the report, Col. William H. Swan, commander of Miramar’s Marine Aircraft Group 11, said that the training flight was “not scheduled, prepared for or authorized appropriately” and both the squadron’s commanding officer and his instructor pilot “provided an insufficient level of supervision to mitigate the risk” they identified before the exercise.

The McDonnell Douglas Corp. MD-88 jets are the oldest aircraft in operation at any major U.S. carrier.
They come with quirks such as glare-prone skylight panels called "eyebrow windows" that were common when pilots sometimes navigated by the stars.
And they're so noisy that some New York politicians, including U.S. Senate Minority Leader Chuck Schumer, cheered when Delta recently pulled the planes from New York's LaGuardia Airport.

一架蘇聯時代的An-2雙翼飛機在莫斯科西南部市郊一個航空展表演特技飛行時，失控墜毀，機上兩人不幸罹難。

An-2雙翼飛機於二次大戰後投產，1947年8月31日首航，直至2001年才停產，主要用作運輸或噴灑農藥。
今次飛機在航空展表演，正正為了慶祝An-2飛機首航70周年。
