據《紐約每日郵報》報導，事發當天2名持槍歹徒闖入休士頓一處民宅，綁架47歲屋主瓦爾拉德瑞斯（Ulises Valladares），隨後家屬接到勒索電話，自稱是墨西哥毒品犯罪集團成員，要求2萬美元贖金。
康德地區警方連忙尋求FBI協助，探員依照發話位置鎖定歹徒所在處，加上男童描述的五官特徵，鎖定38歲嫌犯桑切斯（Jimmy Tony Sanchez）以及42歲嫌犯康寧曼（Nicholas Chase Cunningham)及其35歲女友海斯（Sophia Perez Heath）。
FBI探員埋伏後決定突襲，當時3人均在屋內，沒有做出任何抵抗，但其中一名探員卻因不明原因扣下板機，直接擊中被膠帶綑綁的人質。雖然FBI立即發現誤擊目標，但瓦爾拉德瑞斯送往醫院後仍不治身亡。

FBI 的說法是當時人質上前搶奪探員的步槍，現場光線不良，且為了行動安全沒有開槍燈，探員無法辨識人質身份，為了制止開槍才造成誤擊。

The agents were using a tool to break a window while other agents entered the front of the house.
Acevedo said one of the agents accidentally dropped the breaching tool inside the house and began using his M4 rifle to break the window.

Valladares was next to the window, his hands bound by duct tape in front of him, Acevedo said.
He said Valladares grabbed at the rifle and started to pull the weapon from inside the home.
When the agent feared he may lose control of the rifle, the agent fired two shots, apparently not aware that Valladares was the hostage.
He was struck by one bullet.
